Midwife - first contact

Pregnant and need to book with the midwife?

Please download this form, complete it and return to the practice either by dropping it off at reception, in the postbox or usual post. Once the midwife has received this, they will be in direct contact with your to book your first booking appointment.

New baby registrations - make sure we know!

Usually, the practice is notified by the hospital of the birth but sometimes important information may be incorrect, missing or delayed. Often parents may not have chosen the name at the time of birth. In these cases, registrations are logged under "baby" surname. This can cause problems or delays with registrations.

It is helpful for new parents to notify the practice directly when babies are born and to complete a new patient registration form (GMS1 form). You can download these from our website or google GMS1 form. Alternatively, you can complete these forms at reception. 

This ensures that all the details such as the baby's full name and address are accurate.

Immunisations

A member of the team will be in contact with you to book your appointments for your child's first immunisations, this usually is around the same time as your 6-8 week postnatal check.

6-8 week postnatal checks

Postnatal checks for you and your baby are important and also are scheduled around your baby's first immunisations. We aim to perform the 6-8 week check at the same time as baby's first immunisations, but mum may hear about her postnatal check before this time.

You will be contacted by the practice to arrange an appointment with a GP close to the date you and your baby become due for your checks. We have dedicated team members who deal with these appointments. 

If you feel you are overdue or you have any questions regarding these appointments, please contact the practice and ask to speak to one of the team who deals with the 6-8 week checks.
